Use the following outline for submitting Waco Foundation grant applications.  Please refer to the Application Requirements Page for specific formatting and instructions.

I.  ORGANIZATION INFORMATION – 3 pages maximum

  1. Briefly describe the history of the organization (including the year it was established) and the organization’s goals.
  2. Describe how the proposed project relates to the organization’s mission. 
  3. Describe how your organization is collaborating with other organizations in the community.
  4. List your organization’s board members, their professions and board term (if extremely long, you can attach this as an attachment instead of listing – just answer the question as “see attached”).
  5. List all executive staff members and all staff related to the project/program, their titles and salaries.
  6. List all previous Waco Foundation grants (competitive cycle grants only – not donor advised grants).  Include the date of the award, the amount, the project name and purpose and whether the program is still operating.
  7. List all other local organizations that are providing the same or similar services or programs to your target population.
  8. Does your organization have a Fundraising Plan?  If so, please attach a copy to this application. 

II.  PURPOSE OF GRANT – 4 pages maximum – BE SPECIFIC

  1. Describe the project/program to be funded, including a needs statement.
  2. List the goals and objectives of the project/program – be specific.
  3. Describe the timetable for implementing this project/program.
  4. How does this project/program benefit the local community?  How many people will be impacted? List the zip codes of the population(s) being impacted.
  5. List any known or anticipated challenges (not including fundraising issues) and specify your suggestions/plans to overcome them.
  6. List other local organizations that address the same issues as this project and describe how you are collaborating with them.
  7. Why should the Waco Foundation provide funding to your organization for this project/program?
  8. What is the cost per person/unit being served by this project/program?
  9. Independent of Waco Foundation funding, what other fundraising activities are you doing to make progress towards achieving your organizational goal for this project/program?  
  10. What are your long term sources/strategies for sustaining the project/program and the end of the grant period?

III.  EVALUATION – 2 page maximum

  1. What are the anticipated outcomes of this project/program?
  2. List the outputs that will result from this project/program. (Be specific)
  3. How will your analysis of outcomes and outputs, impact the future of this project and the sustainability of your organization?
  4. Describe how your organization will recognize the Waco Foundation’s support.

IV.  ATTACHMENTS

  1. Organization budget for current fiscal year.
  2. Organization actual vs. budget comparison for previous fiscal year.
  3. Detailed budget for project/program – including all anticipated income streams and expenses.
  4. Fiscal-year-end-to-date profit and loss statement (actual vs. budget) – from previous fiscal year end to present.
  5. Year-to-date balance sheet.
  6. Most recent filed Form 990.
  7. Most recent audited financial statements
  8. Letter on Internal Control.  If a Letter on Internal Control was not issued by your auditors please include a letter from your auditors indicating one was not issued.
  9. Board/Organizational Chart (optional – see Question I.4 above).
  10. Fundraising Plan (optional – see Question I.8 above).
