Grant Making - Application Requirements
Twelve copies of the application must be submitted unbound, without holes or staples, and must contain all of the following information in the order listed. In addition, one copy of the most recent audit must be submitted.
- Application cover sheet pdf file | Word document
- Description of organization (one page) that includes mission statement, year organization began, history and qualifications
- Proposal narrative (two to five pages) that includes description of the project, needs being addressed, population and number expected to benefit, timeline, and future funding expectations
- Evaluation plan (one page) that includes measurable, time specific goals and how data will be gathered and measured
- Itemized project budget, including funding from other sources
- Current and previous year’s operating budgets
- Form 990 from prior year
- 501(c)(3) tax exemption letter
- List of top three employees with positions and salaries
- List of agency’s board members with home mailing addresses and professions
- One copy of recent audit
Application Submission
Your application must be received at the Waco Foundation’s office by 5:00 p.m. on the due date. If the date falls on a weekend, it is due by 5:00 p.m. the following Monday. Your application may be mailed or hand delivered to:
